Immunization Record Request

Get a record of shots or vaccines.

The City maintains a record of immunizations administered in New York City to children and adolescents through age 18. All records are retained permanently.<br /><br />Immunization records are available for children born in New York City after 1996, and for adolescent immunizations after 2005. In addition, the City may have a record of an immunization given to a child, adolescent, or adult if the vaccine was received in a City clinic after 1988. As of January 1, 2015, New York City Health Department clinics will adopt new health insurance and billing practices in order to comply with State law and Medicaid rules. If you are 19 or older, immunization clinic staff members will ask for your health insurance information or a sliding scale fee based on your income and family size. If you do not have health insurance or cannot pay the fee, you will still get services. Your visit remains confidential.

Cooling Tower Inspections for Building Owners and Managers

Building supervisors or property managers who control buildings with cooling towers are required to disinfect all those towers within 14 days of receiving an order in the mail.

<p>Building supervisors, property managers or others who own or are in control of buildings with cooling towers are required to disinfect all those towers within 14 days after receiving the Health Commissioner's August 6, 2015 order in the mail.</p> <p>If you are a building supervisor or property manager of a building with cooling towers, you can get information on how to clean the cooling towers at the Department of Health website.</p> <p>If you manage or control buildings with cooling towers:</p> <ul> <li> You must disinfect all of your building's cooling towers within 14 days of receiving the order in the mail. </li> <li>You must also keep records at the building or buildings you own, showing that all cooling towers were disinfected. If City officials ask for these records, you must provide them by fax, email, or to the City official personally immediately.</li> <li>If the tower was already disinfected within the past 30 days, you can forgo another disinfection, but must provide the records of disinfection to the Health Department if requested.</li> </ul> <p>Building owners must hire environmental consultants with demonstrated experience performing disinfection using current industry standard protocols. You can file a complaint if you suspect fraud or deceptive practices related to the provision of inspection, sampling, cleaning, or disinfection services for cooling towers.</p>

<P>You can report that a fence is missing, damaged, not secured, or does not comply with regulations.</P> <P>All construction sites with an excavation require a solid 8 foot continuous plywood fence around the site. Once excavation is complete and the building is out of the ground, a solid or chain link fence is not required if the site is graded and the building can be locked up.</P> <P>In residential locations, fences are not required between properties. The maximum height of fences in front, rear, or side yards depends on the zoning regulations for the property. In lower density districts (R1 through R5), fences can be a maximum of 4 feet high. Fences built before April 30, 2008 can be 6 feet high. </P>

NYC Cool Roofs Program

Program to help building owners reduce energy costs, greenhouse gas emissions, and local temperatures by coating their roofs white.

NYC Cool Roofs is an initiative to mobilize volunteers to coat the rooftops of participating buildings with reflective, white coating to help reduce cooling costs, energy usage, and greenhouse gas emissions. The initiative will help the City meet PlaNYC's goal of reducing the City's greenhouse gas emissions by 30% by 2030. <p>Since most New York City roofs are dark or black in color, they retain the sun's rays as heat, increasing summer roof temperatures and energy demand. Dark roofs also contribute to the 'urban heat island effect,' a phenomenon where the City can be 6 to 8 degrees warmer than surrounding suburban areas. Coating all eligible dark rooftops in New York City could result in up to a 1 degree reduction of its ambient air temperature.
